I am currently a participating provider with Aetna, Blue Cross/Blue Shield, Cigna, GHI, HIP, Emblem, Medicare, United Health Care, NYSHIP, 1199 as well as several other plans. Healthcare reform has had a positive impact on many preventative wellness services such as nutrition counseling. As a result, your health insurance may cover the entire cost of your session. However, in some cases, even with an excellent insurance policy, nutrition services may not be a covered benefit.

It is the Patient's Responsibility to verify the coverage provided by their own insurance. You will be billed for any services provided that are not paid by your carrier.

To verify your nutrition coverage, please call the 800 number on the back of your insurance card and ask to speak with a representative from member services. Use my name as the provider and our address location in Patchogue. If asked, you can provide them with my NPI # 1205029956
